If it is casually, it is a sign of affection. Anything can be an insult if said in a derisive way. However if they are just saying it casually, it is kind of cute.

they think that u think u are the **** and they may agree or just since they are saying it casually and not in mean way they may think the same way or just mean they know u think that so they call u it.
