They are icons that you can give to the people on your messenger friends list. Depending on what IM your using you can change them for your friends, or they pick their own icon.

They mostly associate with aim and aol but in yahoo's case it is called an avatar. It's what u have as a picture. Smiley faces used to be very popular but that's just an example.
