Check his bloodpressure? Does he have heart problems or angina? What's his age, chloresterol level? If the heart is okay, and the major arteries, then lungs, lymph nodes, lumps that could be cancer, (the lymph node in the armpit will be effected), and then look at it just being a pulled muscle. Left arm pain can be an early sign of stroke. When dealing with life you eliminate the worst possibilities first, and the heart is nothing to mess with.

Possible pulled muscle, has he lifted anything heavy or used that muscle and not use to doing so? Does it hurt when he takes a deep breath? Does he have a cough? If this is the case it may be in his lung or even ribs....if it continues to bother him he should see a Doctor.
