Features I need are:
1. Structural carbon steel construction for welded installation in 12-inch steel pipe.
2. One swivel axis, with at least one single row ball bearing with hardened steel tracks.
3. Sealing surfaces shall be stainless steel to protect against corrosion.
4. Seal replacement, ball replacement, and relubrication is possible without removing the swivel from the pipeline.

Does such a thing exist?

10" is the largest I've seen but it takes a real man to handle it- 4 and 6" are much more common.

The opw link is a good lead. If it exists you will likely find it through a petroleum equipment manufacturer. Also try EBW (enterprise brass works) and Emco Wheaton.
