My daughter has had TMJ for 3 years. She gets ear pain and 'pain spikes' in her ear. We see a phsical therapist, a chiropractor, she has a bite brace, eats a soft diet, doesn't grind or clench her teeth..
So now we are at the end of our rope! I know I am grasping at straws but who else can we go to? We already see an ENT who said he can't do anymore. She sees a dentist again Thursday.
But my question to you is this: Oral Surgeon? Mayofacial surgeon? Who will have the answers? At this point all I really need to know is where to start and what treatments are available now?

Her TMJ is really severe. I would go to a Maxillofacial surgeon:

When I had TMJ I tried everything but eventually, after everything, I realized the problem was stress and probably the by product of an old automobile accident. A Dentist did a bottom jaw guard for me to sleep in and it was partially successful. I also had acupuncture and that seemed to help a lot too.

Often, a TMJ patient can benefit from a bite adjustment.

It is not clear what is the basis of your statement that your daughter has the TMJ problems. However, it can be addressed by OMFS (oral & maxillofacial surgeon). It may also be worth searching for options overseas (dubai, india, thailand, singapore, etc) by sending them reports in email. This form of second opinion might be available at no charge, while you would also get an idea on the cost of treatment there. The possible search could be TMJ TEATMENT IN DUBAI/SINGAPORE/INDIA.

THE ANSWER IS A DENTIST WHO SPECIALIZES IN TMJD.THIS MAY BE AN ORAL SURGEON OR A PROSTHODONTIST. AN ENT MAY NOT DO ANY GOOD BECAUSE THE DOCTOR WHO TREATS YOUR DAUGHTER MUST REALLY UNDERSTAND THE RELATIONSHIP OF THE JOINT, TO THE MUSCLES, AND TO THE TEETH. A PT OR A CHIROPRACTOR HAVE LITTLE OR NO EXPERTISE TO TREAT THIS TYPE OF DISORDER. CONSULT WITH AN EXPERT IN TMJD TO FIND ALL YOUR OPTIONS.

Has she tried a night guard from her dentist? How do you know she is not grinding or clinching while she sleeps? I have TMJ, my jaw pops, etc. I have a night guard and it worked great. Don't chew gum or open really wide. My jaw still pops but rarely any pain!!

Chiropractic treatment hasn't been able to cure her?
How long did she treat?
I work at a Chiropractic Center and we have cured many TMJ cases.

Get surgery on your jaw, that what I was supposed to do, but it went away!
