God knows and sees all.
He sees your neurons firing, so yes He can read your mind. He also wouldn't have to, since He knows ahead of time what you will do, better than you yourself. (He was there your entire life and knows your experience and thoughts and actions). It's like God is watching a timeline of you, or more like designed one for you, and knows what paths you will freely choose.

Yes, God knows each and everyone of His children and knows their hearts and minds and most innermost thoughts. He created us all each one different and unique and loves each one of us more than we could ever even begin to imagine. Even when we fall into sin, His love is constant. He doesn't like our sins, but He does love us despite them, and He waits for us to confess and receive His forgiveness and mercy.
