Usually, that's up to the mother. When the baby is born, Mom will be asked which name she would like on the birth certificate. She can choose to use both, either, or neither of the names. Most reasonably civilized places have a policy that it must be the mother's decision alone, as the people who handle deliveries understand that mothers are usally the primary caregivers and may have very good reasons for not wanting their children to have their fathers' names.
Of course (and thank God!), most parents are usually able to agree on appropriate names. Traditionally, only the father's name is used. This can vary from situation to situation, though, so there's really no reason to apply any pressure one way or the other.

You may give your baby any name you wish on his/her birth certificate, including the last name. The parents pick which last name the baby gets. They may choose to go with a hyphenated last name comprised of their 2 last names or may choose a completely different last name if they wish. Most commonly, the baby will be given the last name of the father if his last name is different than the mothers, but this is not always the case.

Usually the fathers name, but it is up to you and the father not the doctors or legal system or anything. If you want the baby to have your last name simply tell the doctors that. Also, if you are unmarried but are in a relationship with the father, the baby is usually given the name of the mother, since you aren't married. In the end it's entirely up to you though.

That is solely up to u. Are u married? If u are married then u have to give the baby the father's last name by law. If u are just together, then it is up to u whether or not u want to give the baby his last name or not. U need to do what is right for u to do, I don't know Arizona's law that well but, I am sure it is the same for all states. Well good luck in your decision, Best Wishes With The Baby!!!!!

My son was born before me and his father married. While we were in the hospital he was listed with my last name. When we filled out the birth certificate, he was given his dad's last name. However, it could be either last name.

Whichever you choose together. It can be neither if that's what you want. It's totally up to you and your husband. There aren't laws stating which name you must use.
