Only your doctor knows how. I suggest you don't try to forge a signature or falsify a prescription. This is a felony in the US and will send you to jail.

Ideally a prescription should be written legibly and contain all of the following. 1 Physician's Name, Address, and Telephone #, 2
Physician's Controlled Substance # (if required), 3 Date, 4 Patient's Name, Address, Telephone #, 5 Medication Allergies (this should also be on file at the pharmacy), 6 Generic Name of Drug, 7 Brand Name (if brand is required and some areas require that dispense as written or brand necessary be written if the brand is required), 8 Dose, 9 Directions, 10 Quantity (I write the word out rather than simply putting a number), 11 Refills, and 12 Physician's Signature. I comply with nearly all of the above but my prescriptions are far more detailed (and yes they may be easily read) than the vast majority of prescriptions. The medical literature suggests that about 1/3rd of prescriptions are considered illegible, 1/3rd are transcribed by the pharmacist with error, and 1/3rd are transcribed correctly - at least in the United States. California, Florida, New York, and Texas also require a special prescription pad for Schedule II drugs which may not be refilled. Other medications may be refilled either 5 (for a total of 6 months) or 11 (for a total of twelve months) times. If you would like a list of Latin abbreviations write to johnerussomd@jhu.edu and I will send them.

Only a doctor can write a prescription. Each one is assigned a DA# that is logged under their name, so prescriptions can be tracked.
If you forge one, you are looking at a huge fine & jail time.
The ones that forge pain meds ruin it for the patients that are really in pain.
See your MD for medication.
