If I were a therapist and the child of one of my clients made Barbies have sex...
If the child is like six, I'd begin to wonder about the parents. And the surrounding family and friends. Parents may not be causing problems, but they may be ignoring whatever could be going on.
If the child is over ten, exploration of sexuality is more probable. And a certain disassociation is normal of young people (and people who watch porn).

I'd definitely investigate the ABUSE angle. Find its cause, nip it in the bud, and stop a chain-reaction before it's made. Violence feeds on itself, after all.

If you truly see a child that exhibits this sort of behavior you should contact child protection services and report a suspicion of child abuse/ sexual abuse. This type behavior is extremely common in children who have been exploited in this manner, and is highly unusual in children of an age to play with Barbies to begin with. Even in older children, it is not appropriate and indicates a problem that needs further investigation. I wouldn't just ask the child, I'd tell the authorities.

if you mean question the child no but i would question the therapist i would need some more indepth in fo on your question and the events surrounding it
