Tell how long you have known your friend, and in what capacities (work, personal, family, etc.) and then, being as professional as you can, describe the wonderful qualities about your friend, particularly surrounding what the letter of recommendation is for. For example, if your friend was asking for a letter of recommendation for working in a day care, you might want to think of times that you have seen your friend with children, when you have known them to be responsible, when you have known them to go above and beyond. Remember to be honest, but also know that if your friend has asked you for a letter and you have agreed, there is probably an understanding between your friend and yourself that you will say nice things about them =). And if you can't say nice things about them, don't agree to write the letter. For what its worth. Hope that helps.

State how you know the person, for how long and what that person has accomplished for you (but only a brief synopsis, not a biography). Make a statement at the end that in your opinion, you have seen this person to be an asset to your company and that you would welcome the person back as an employee.
