First would be your basic information (name, date of birth, phone number, and address). Then where you were standing when the incident occurred. Then in your own words what did you actually see. Witnesses are ruled out usually in this area. If you did see the incident take place, then it would be the fact of attaining certain noticeable features of the suspect or suspects, how did they depart the area (car or on foot) and which direction of travel. And most important could you identify them.
Also things picked up is if they (suspects had an accent, or a slur to their speech, color of skin, type of clothing, and what was actually said by them). In these types of crimes the basic information is logged and checked with previous incidents for matches.

It depends on the level of investigation. It is always best to let your witness talk. For example, if you limit your witness to only talking about the incident (robbery) they may not tell you the a teller was kissing the robber in the parking lot before he came in. Just think, what would someone do? Where can the investigation lead. Interviews are just listening then getting more information on what has relevance. People don't always volunteer important information.

If you noticed any unusual or permanent marking ( tattoo's , piercings, body hair, certain facial movements, eye color, mannerisms. ) Anything to pick out the suspect.

Any distinguishing characteristics. Skin tone, scars, eye color, what he was wearing, did he walk funny(limp), how did he talk(accent, speech impediment), anything that would make him stand out in a crowd.
