It comes from Nabokov's novel, _Lolita_. In the book, Lolita is a young, very attractive girl who has a strange affair with her step-father. So the term has come to mean an underage, but very sexual girl, usually one who tries to seduce older men. Note that it's also a nickname for Delores (which is the character's full name in the book; she also goes by Lo or Lola).

It depends on the context you use it in. In the states in commonly used to describe some one who is like a character in a book, named the same.
However, in Japan and places where this fashion explosion has taken off the term Lolita is used to describe a girl or boy who dresses in a more conservative fashion generally based on the Victorian Era or the Rococo Era. These girls and boys do not use the dress as a way to express sexual promiscuity but actually the very opposite. It's an extremely elitist society not unlike the society of the Victorian Era.
Girls are required to wear skirts that reach at least their knees, where bloomers and panniers/petticoats, socks, and blouses that have colors and cover their necks. Also a head dress of some sort is generally required. There are several different types of Japanese Lolita.
