I see by some of the answers that people don't seem to know much about first communion. Does Carol M. really think that you friend is NOT already a Christian!! Honestly!!
Does she have a rosary? A nice rosary, a bible, a book about the different Saints with nice pictures. If she were older than 7 or 8 a singing version of the Psalms, like the Grail Edition would be nice.
Marbledog's idea is a very good one too. Sorry I didn't think of it. I have things like that given me years ago and I always think of the person that gave it to me when I wear it.

Communion is symbolic cannibalism. The girl is symbolically eating the body and drinking the blood of her dead god. I would, under these conditions, give her the only appropriate gift for the occasion, a shrunken Jesus head.

Depending on age and denomination. If Catholic, there's a gift pack you can buy her that has a rosary, prayer book, etc. If not, you can get her a cross pendant/necklace, Bible, anything that she can keep for a long time and it will help her remember the first communion.God bless!!

A medallion of her patron saint would be nice. Most Catholic gift shops stock a large variety of patron saint medallions cast in pewter or nickel. On a sterling silver chain, it makes a personal, attractive gift and is relatively inexpensive.

A silver or gold cross necklace. An easy to understand Bible. Try looking at a Christian store. There are tons of different items to choose from.

White rosary, small children's Bible.
My favorite is this:
If the child was given a Saint's name at Baptism....give him/her an icon of that Saint. Have it blessed and dedicated to the child/signed by the Priest who did the Baptizing

for my first communion I gat a childrens Bible a rosary and a childrens book on how to pray the rosary
