bleeding per se does not cause pain. pain occurs because of accumulation of bllod, or body's response to the blood-inflammation. so pain will depend where the bleeding occurs, how fast and how much. so slowly occuring bleeding in abdominal cavity may not cause bleeding or may cause it late. obviously for the same reason bleeding in the joints and brain would be very painful.

Depends on where the leak is, and how much. Leak into the gut, it may be painless. Leak into the brain, or a joint, it may hurt like Hades.
"Internal" covers a lot of ground.

Absolutely!
In fact, bleeding into joint spaces is characteristic of hemophilia, and is very painful. Tsar Nicholas II's son with hemophilia was plagued by this symptom.
