Most of what have been said is pretty standard for anyone who comes to PR. These are things travelers normally dont get to see.
If you want to really do something different check this options:
Aventuras Tierra Adentro - these guys can take you into huge cave systems, rapelling in amazing places, body rafting. Pretty intense.
For something less physical:
Island Adventures Bio bay echo tours
Don't go to the bioluminiscent bay in La Parguera, very long drive, its contaminated with riverhouses and they tour you in a big boat. Go to Vieques or Fajardo for cleaner ones. Vieques is a tougher trip but everithing is unspoiled.
Go eat seafood where the locals do. A lot cheaper and you wont get sick. Do try the tostones, alcapurrias, bacalaitos, pasteles (in Christmas time) and any other weird food they offer you. You might gain 10 pounds in a week though.
For a great local food experience check if a local can take you to El Meson de Melquiades in the town of Cayey (very long drive in the inner mountain zone). Dont attempt to go by yourself unless you are willing to ask about a dozen times for directions.
The beaches in Vieques and Culebra are breathtaking.
Be sure to come in during new moon to appreciate bioluminiscent bays

There are some people that already gave you very nice ideas. You can also check Que Pasa Magazine www.qpsm.com. That is the official magazine for the tourism company in Puerto Rico. There you will find the address to the places suggested, as well as restaurants, hotels, transportation, etc.
Have a nice trip!

check out the Horned Dorset in Rincon if you want a change. Not many people visit the west side of the island (most go to San Juan) but you will catch some beautiful sunsets there.
