nope; never! Rashad Khalifa though (a man who interpretated the Quran and discovered an amazing mathematical formula behind it) did try to omit 2 small (somewhat neutral) verses from the Quran because he thought they were unauthentic. This causes A LOT of controversy and he was killed (while I don't believe he had a right to try and change the Quran he shouldn't have been killed). He has a very loose translation of the Quran that might actually appeal to a lot of westerners :) too bad the Bible was tampered with. Good question though!

Yes! What happened was that the Quran was not initially written down. Muhammad spoke primarily to slaves and the poor (who could not read or write).
Then Muhammad's religion started spreading since he was a chrismatic speaker and the people of the area needed something to unite them.
Some people started writing down what they heard Muhammad say. However, what Muhammad said changed somewhat each time he gave a sermon (for example, once he claimed he was merely an apostle (follower of God) then later he claimed to be the "Last Prophet" (final authority on God's revelation) then there was the remark that God created man from a clot of blood and then at another time Muhammad claimed God created man from dust etc.)
Sometime after Muhammad died, Caliph Uthman (the third one after Muhammad died) decided that he needed to compile all the memories of those who had heard Muhammad's sermons many years earlier and the notes that had been written into a complete written version.
Uthman formed a committee of about 10 people who decided what would and would not go into the Uthman version of the Quran. Any notes or writings that the Uthman committee did not like were destroyed.
Also any known versions of Muhammad's sermons were hunted down and also destroyed so that the Uthman version would the only version. Uthman's version was written in a manner to support his tribe or clan. For example, his family was the primary owners of Mecca and the surrounding areas. Prior to Muhammad, the people of the area were pagans. One of their traditions was a pilgrimage to Mecca. This pilgrimage was financially beneficial to Uthman's family. So in the Uthman version of the Quran, the pagan tradition of the pilgrimage was included. This insured that Uthman's family would continue to get this "tourist" money.
Allegedly one of Muhammad's wives (Umar) allegedly had a copy that survived the "burning of the Qurans."
There are rumors that the Umar Quran was discovered by the British Army in Turkey during World War I and is being held from release since it is remarkable different than the current version in certain aspects. Many Muslims today mistakenly believe that the Uthman version is the only version and has not changed. However, that is merely a religious myth.
Anything that is contrary to this religious myth causes some Muslims to get highly upset.

Not that I'm aware of. At one point in the Qur'an's history, all versions but one were burned so there would only be one version out there. It was done shortly after Mohammad's death, I believe.
