Actually it's an idiom. Welcome to the English language which has more Idioms than almost any other language. There are whole books written on English idioms. Which you can understand is one of the things that make English one of the harder languages to learn. Teach a person how to say 'go to bed', and they're fine. Now tell them to 'hit the rack' and they have a blank look on their face.

Another way of phrasing it would be 'full of herself'.
The others were right, she has a big ego to be exact.

It doesn't literally mean that she has a big head- its a proverb. it means that she has a big ego, or in other words she loves herself & thinks she's great....another proverb for this is Mary thinks that she's "the bees knees"

no it means whatever mary is doing, she probably getting a bit carried away with it
