If you have any holiday owing to you, then I'd just ask for that day as holiday. Unless you always provide exact details on what you'll be doing on a day off, then you don't need to provide them with more details.
They can draw their own conclusions and you don't have to lie and feel awkward.

Don't call in sick... you just wont look sick enough the next day.
Make it either a dentist's appointment, or a doctor's appt.
Car care is not really a good excuse to skip work.

Can't you schedule it for lunchtime? Otherwise I'd say doctor's appointment or sick day, depending if you've missed any work. I've actually had people willing to interview me after hours, like at 6:30 or 7:00 pm or even 8:00 am.

Honesty is the best policy at all times. However, it can be stretched.
When you call your employer, let them know that you have a personal matter that must be attended to today. In most cases you will not be asked what it is because personal matters are no one's business other than your own. Plus it appears that you or your employer know your time is limited in this job. It's your life, take care of business first.

Just ask for the day or a certain time of the day off for an appointment. They don't have to know what type of appt you are going to. Or don't even say you are going on an appt, but just want time off.

Dont say you have an appt., that makes it obvious. Just say you are sick! You dont feel good. IT doesnt have to be specific.
