do people pat you down before a regulated drug screen?

Yes To make sure you aren't bringing a container, bag etc with someone else's urine in it..and no bags are allowed.

I have taken drug tests and never been patted down. But they shut off the sink water and the toilet with electric switches. They can tell by temperature if you substitute for your urine sample. In the service I had urine tests where they watched. The best and most reliable way to pass a drug test is to not do any drugs.

I've only had one and they didn't seem to care what I did. I left my bags on a chair outside the bathroom just because they were a hassle. I had tons of stuff in my pockets and I could have been carrying anything but they barely looked at me. They gave me the bag with the cup and pointed to the bathroom. The test was pretty important because if I failed, I wasn't going to be admitted to a particular treatment program at a hospital (not a criminal thing). Could be different elsewhere though.

I don't know about that, but some will go in the bathroom w/you while you pee in the cup, they won't watch, but they will be in there to make sure you don't try anything.
