I need a letter to give my employer about a sick leave?

I will be gone for surgery and need to inform my emplyer. Any sample letter?

Your doctor should write it for you. These days though gossip is amuck even in HR departments. this is what i would ask the dr to write:

Patient X is having surgery on x date and will be out until x date recuperating from the surgery. Please contact me with any questions that I can address without violating HIPPA regulations
