The physical form can be picked up from your school's athlethic director. Some schools have a special night at school where they offer physicals for the students who are going out for sports. Usually, these are in May for the following school year. Any pediatritian will be happy to fill out the paperwork for you. Be sure to make an appt asap because they fill up fast.

Just make an appointment with your family physician or whoever your doctor is and be sure to tell them that you want a physical, for school. The school gave my son a physical fitness form for the doctor to fill out. If they don't give you one, the doctor should have some forms.

If you are old enough to play sports perhaps it's time to find a family doctor rather than a pediatrician.
But either can give you a physical for track.

Yes and you need first to have a phisical or if u had one just ask the doc 2 fill it out (he will for a small charge)

make SURE you ask the coach if the school has any forms they want the doctor to fill out for them.
When I did track I got a form to give to my doctor, and she filled it out.
