This is what will happen:
1) The scientific law of the conservation of matter states that matter can be neither created nor destroyed. In other words, everything that exists in this universe will continue to exist in one form or another. In fact, in your body right now could be atoms that are billions of years old -- the remnants of stars that blew up long before the Sun in our sky existed. When you die, the physical matter of your body will continue to exist. In a sense, "you" will be one with the Earth and no matter what, "you" will always exist in the universe in some form.
2) The scientific law of conservation of energy states that, like matter, energy can be neither created nor destroyed. When you die, the energy of your body will be liberated from your body and into the cosmos.
These are both scientific facts, by the way. They WILL happen, regardless of what anyone believes about religion or philosophy.
3) As for what happens to "you" (i.e., your soul, consciousness, etc.) -- no one really knows. If anyone tells you they do, they're probably wrong (and even if they're right, it's a lucky guess). In the end, you can't know. You can have beliefs about what will happen, beliefs that are effectively the truth to you personally, but there is no certainty.
My advice to you is to not worry about it. If you have any religious beliefs, it probably couldn't hurt to live by them, but use those beliefs wisely. The whole point of religious belief is so you can be a good person while you're alive, which in the end is what really matters. You're only given so much time in this life -- so don't waste it worrying about death, use it to live. Don't take my word for it or anyone else's either -- figure it out for yourself.

Ever gone to sleep and you wake up the next morning and can't even remember when you fell asleep? It was quick, quiet, peaceful, and many times, not even a dream.
Death is the same way. Most will not believe me mainly because that's not what they want to hear. Do you want to hear the fabrications and stories of priests, or ministers, or even those representing Allah? Or do you want the truth.
The Bible over and over again states that death is no more then a good nights sleep. Jesus said he went to wake up Lazerus after he fell asleep in death. It states that all thoughts, desires, etc., parish the day you do.
Do you go to heaven...highly unlikely. Heaven was made for angels and not us. There are a number of humans going to heaven but I know I will not be one of them, chances are...neither are you.
Do you go to Hell? Think about it. We have a God that loves us so much, do you think he would send you to Hell, or if he's real pissed, maybe to send you to sleep. Kinda like a father or a mother might do to an unruly child..."Go to your room, turn off the lights, go to sleep!" God is the same way, only difference is that you will never wake up.
So what will happen if I die. If you allowed God to lead your life, accept Jesus as your saviour, and all that other stuff, you will have a chance to liver forever on a paradise earth. Actually that's how the story started in the garden of Eden...God, being so powerful and all, maybe you can fathom a guess as to how it's gonna end. Do you think it's your choice or His?

British Esquire in that publication's infancy 20 years ago did a long-ish piece on precisely what physically happens when we die - the various paisleys that emerge telling pathologists and coroners what time precisely death occurred. Go to a medical library in search of similar material, which should at least allay your concerns about the physical side of death. Focus on those questions, which can be answered with some certainty, and do continue taking your medications for schiz. if such a diagnosis has been made. That's your best plan for quieting the voices you describe.

I don't have schizophrenia; I'm sorry you're dealing with that on top of worrying about death.
Sadly, no one knows because no one has come back to explain. However, there are people who believe they have seen Heaven or they have seen Hell. You could listen to them.
There are people who remember previous lives. Some children have episodes (which can be confirmed) where they remember the ends of lives of people who passed away before they were born -- people the families never met nor knew before the episodes occurred. You could listen to them.
There are people who go with the theory you wink out like a light and you end up nothing and no one. You can listen to them, too.
For me? I follow the idea that I will keep coming back, challenged again and again by the unlearned lessons of my previous lives and challenged again by the learned ones. If I succeed in finding a happy medium in life, then I get to leave the cycle of rebirth and cease to be. Considering, in my belief system, that life is suffering that we inflict on ourselves (though in your case, it seems you've got more on your plate than the average person), the peace of nothingness is very pleasant.

Don't worry, energy is neither created nor distroyed. At its best, it will only transform into another form.
I have some spiritual beliefs but I will not go into them as I am not sure of your background, but I will mix a bit of science with some ideas to try to give you a hint about your end.
If you die and you're transformed into sand.. then at least you know your end is sand. Examine it..
What is common between you and sand? That you're both energy and conscious, however sand does not have the ability to keep track of time and memories, which means sand lives in the eternal time, unlike humans who are trapped in time through their perception.
Conclusion, if your end is sand you're in a much better position than you are now, eternally living.

There is only one answer and it is a question. What do you want to happen to you when you die?
Having faith in something whether it be reincarnation, heaven...whatever...will ease your mind. All we can ask ourselves is what do we want to happen to us, and then believe in it, because there is nobody alive that knows the right answer.
I believe afterlife is like a feeling of complete joy. I can't explain it well, but an analogy would be when your body hurts so so bad, and you take a percoset, and it feels like you are just lifted out of your pain. OK now think of that but so painless that you cannot imagine... Our bodies and our minds weigh us down even if they are good...when they die, we are weightless with no struggles, no stress (good or bad), no anger, and what is left...joy

I beleive that our minds are not advanced enough to process non-existance. I'm personally open to ideas and theory, but don't believe anything until I see it.
The religious nuts with undoubtedly start posting answers about their religions. I believe these are just people that are just as scared as you that they get join churches and swallow garbage logic out of fear.
The bottom line is, it's okay to be afraid. There is some peace you get when you admit you don't know and don't have any control over it. Don't waste your time here worried about where you are going next, just focus on what you want to accomplish here and the rest will take care of itself. Do what makes you happy and don't let the bible bangers ruin your trip; they are just jealous.

I would say stop worring about and start living more.
But if you are looking for some comforting theories on the subject I would read "the Other Side" by Slyvia Browne or any of her books. Even if you don't believe in everything she says (she's a psychic). I find her books calming.

Why worry about it? Its gonna happen but meanwhile enjoy every day.
I think that nothing does, you just cease to exist.
I think that there isnt an afterlife, but there is today and the life you have now which is what you need to focus on
