Advanced maternal age is age 35 or older in the US. It is age 37 or 38 in other countries, like England and Australia. Nothing magic happens at these ages. Instead, it is a general guideline used to determine which pregnant women will be offered prenatal diagnosis for chromosome abnormalities. They use maternal age because there is a relationship between the age of the eggs and the chance that the egg will have too many or too few chromosomes. Women are born will all the eggs they will ever have. As a woman gets older, so does her eggs and older eggs are more likely to have chromosome problems.

At age 35, there is a 1 in 256 (less than 0.5%) chance that the egg will have an extra chromosome 21 that causes Down syndrome. At age 34 the chance is a little smaller. At age 36 the chance is a little higher. The American College of Obstetrics and Gynecology chose the age 35 because they felt that there were enough facilities in the US to support offering prenatal diagnosis to all women in that age group. It was based on a "cost-benefit" analysis.

Sperm are different. Men make new sperm every 60-90 days. Sperm don't sit around in the body and get older. What does happen is that older men have sperm that have undergone many, many, many rounds of replication to make all of that sperm. During each round of replication, DNA mutations occur. By the time a man is in his late 40's or older, his sperm may have a mutation in just the right spot that will cause a genetic syndrome There is also some recent evidence that older men are prone to making sperm with extra or missing chromosomes, but this risk is smaller than compared to a woman in her 40's.

In general, MOST men and women over the age of 35 will have healthy babies. The term "advanced maternal age" makes a woman sound very old and higher risk that she probably is. The silver lining to being advanced maternal age is that you usually get a high level of OB care compared to a woman age 34 or younger.
