I would say these two (both Asian, strangely):
The Untold Story -- it's a Hong Kong horror movie that is VERY unsettling because you see children getting killed and it's very graphic. I've seen well over a thousand horror movies, and honestly most of them were dull or laughable, but that one really got to me. It's not really a good movie though.
The Guinea Pig 2: Flower of Flesh and Blood -- you probably don't even want to rent this (if you can even find it). It's a Japanese movie with extremely gory special effects that look so real that when Charlie Sheen saw it he contacted the FBI because he thought what he saw was real. It's pretty obviously fake if you pay attention, but it's not really a film with a plot. It's presented as if you're really watching a murderer videotape his crime.

I think I was most traumatized by Even Horizon (at the time it came out) and probably by the shaking head guy in House on Haunted Hill (yes, both cheesy films, but I still wouldn't want to encounter that in my closet!)

Poltergeist and any the classic Nightmare on Elm St movies. The originals were classic - the sequels grew tiring.

Salems Lot - "The Master" used to scare me to death as a child.
Worst ever thing I have seen in a horror film though is Cannibal Ferox, thats plain sick.

2-The Shining- those twins scare the cr*p out of me!
