The scariest part for me (a catholic) is that I've read about the case history on which this is based and the excorcist is a true story about a demonic possession and exorcism which was validated by the vatican authorities. In other words, when you watch the movie, what makes it scary is that the events (mostly) actually occurred.
In real life, however, the possession was over a little boy and involved the use of a ouja board to try and channel spirits.

I first saw this movie when I was about 9 or 10 and that was a big mistake - seeing it now isn't as disturbing but because I was so scarred by it as a child I still have that psychological attachment to it as something incredibly scary. Also, the fact that it's based on true events is scary. If you're older, watching it isn't all that bad... but the fact that it happened in reality is what's so scary.

What makes "The Exorcist" scary in my opinion is that to alot of people, it could be "real". For example, a movie about a vampire or werewolf, most people dismiss as not real and pure fiction/legend.
Then you have the psycho killer movies (Saw, American Psycho), etc....but those are too "real" (there are alot of psycho's in the world).
However, the Exorcist has a girl being possessed by the Devil. So most people with a religious background have faith that a devil exists, and this is one of their fears. But no one can prove it or disprove it. So it would be real or not real.
That is what is scary about this movie to me. The possibility that it is real.

Its not that scary but it is very psychological.
Whatever you do, skip Exorcist II and go straight to Exorcist III after watching it. The third film in my view is better than the first storywise. Very underrated film which does not get the acclaim the original film does.

I think it's all relative. I just saw it for the first time a few months ago, and I wasn't scared at all. I think when it first came out, it was really scary, but since then movies have gotten scarier. Really, I think it's more creepy than it is scary.
