Take it from me,I've been there,4 months is a very, very short time. Nothing to make a big deal about.A small encouraging gesture is all that is really appropriate. Some fancy coffee or something like that. The Big Book, or Daily Reflections to reinforce staying clean.

First, it is so wonderful of you to stand by him during his rehabilitation. Many women wouldn't have stuck around that long.Yay for you!
Second, it truly is wonderful that you all are proud of him and want to show your appreciation. However, I would strongly suggest displaying this pride in a big, heartfelt toast, possibly by a few people.
Making his clean record the highlight of the entire party will put a lot of pressure and stress on him...it makes the expectation that he will stay clean even greater, and sometimes high stress and fear of expectations are reasons why people abuse drugs.
I'm not sure if he has the willpower to resist a relapse after 3 months. But it is also very important that you show an appreciation for what he has accomplished. That is why I think a toast is a good idea. It shows your appreciation but doesn't take it over the top. The key is to show love and appreciation without pressure.
Plus, the children present might not know the specifics, or fully understand the situation. A toast will limit their confusion.
As for a gift, I would get him something engraved with a loving message. A simple pocket watch that says "The more time passes, the more I love you."

You should be proud of his effort but its not something you should reward him for. He is an addict for life. It doesn't go away because of a three month stay in rehab. If you went to any kind of counselling with him you would know that he should not be rewarded for something that he should have been doing all along. Showing your love and supporting him through his rehab and standing by him now is REWARD enough!!
Make sure there is no alcohol at this party and make sure all of your guests do not bring alcohol into your home. Its a sign of respect for your husband.

Stock in a company !!
Figure out how much he has saved from not doing drugs and invest it for a bright future. Why not have something to show for it rather than some trinket that will just lay around... BUY things that will make you/him more money and life will get better everyday !!
