It depends on what the contractor will charge you per square foot. Get different bids and look at some of their previous jobs before you make a decision.

For materials you'll need to know the number of cubic feet. There are several grades of cement. When you know what type of cement you want,figure up the number of cubic yards needed,and call your local redi-mix supplier for prices per yard. :-)=
