I probably would at first, but not for that long. The people at Nike will eventually get used to your look. I have some piercings and tattoos, but the only thing anyone can see besides my ears is a diamond I wear in my nose. Then I also don't want to advertise. Sign a limited contract; when it expires refuse to resign unless that is taken out of the contract.

Depends on how much they're paying. For the right price, I'd grow dreads, get piercings and tattoo the Nike swoosh on my chest.
(I had to attack this question from the other end, since I lack all the features you mentioned)
