I have adopted brothers and sisters and when you get adopted it's like you are being born again. They issue out new birth certificates and at that time you have the option to change frist and last names or you can leave the name that was given to the child.

that is up to the person getting adopted if they are old enough to make the decision usually 14 will do it

Depends on their age or and if the child agrees to it.
