Most likely she inherited the mange from the mama dog. Dogs of either sex who have this condition should NEVER be bred, as it is passed on to the puppies!! These are not healthy dogs!
Take her to the vet to get treatment. Get her spayed, as that helps control the outbreaks, probably due to controlling hormones.
Put her on a very good diet. Like the ones at http://www.frrco.com/121668 This will boost your dog's immune system and help ward off re-currences.
Also, be aware that sometimes demodex mange is indicative of an underlying medical/health condition. Your vet will need to do a thorough screening to rule out any additional problems.
If you got your puppy through a breeder, advise her or him about the condition. Also, ask the vet who you can report them to, as they should not be breeding dogs with this known condition.

All dogs have demodex, a lowered immune system can bring on an eruption. if is in a puppy ,chances are the problem will go away on its own,(small spots in different areas),if is generalized , it requires a process of dips, to control it. Also a low absorption of zinc will affect the coat, and show signs like demodex, (Zinpro is a product you might try).

All dogs have demodetic mites present in their skin. Her immune sytem should keep mange in check but sometimes they multiply and cause hair loss and rash. This particularly happens about puberty. Take her to the vet and they should give you a shampoo to use every 2nd day. If the mange doesnt respond get a product called advocate(its by bayer) its a spot treatment available in 3 months or 6 months worth from the vet. Its a bit expensive, but it does mange,fleas,ticks,worms and heart worms which makes it worth it.

she might have mange, or a skin infection or allergies. Some dogs are more prone to skin problems than other.

Take her to your vet who will do a skin scraping. It is likely that she also has mange.
