I've seen insulated cooler/warmers. In fact, my friend has one. It plugs into the cigar lighter of your vehicle and keeps food either cold or warm, depending on the switch setting. They work great. One night, we went fishing and stopped at a McD's and got a bunch of cheeseburgers. We put them in my friends warmer, and ate them around 2:00AM. They weren't dried out, and were just as good as if we had just bought them.

They used to run around $40 to $70, depending on the size. For an additional $30, you can get an AC adapter that lets you plug the cooler/warmer into a wall socket. I've seen them at OSH hardware stores and sporting goods stores. Trust me, you'll get a LOT of use out of it.

If you don't have a good thermally insulated container to keep it hot then you have to carry a portable stove with you.

use a small gas/white gas stove

they are pretty cheap...from $30-$60
