Contact the offices of the doctor that was your primary care physician and all the specialists you visited and ask for your records to be transfered to your new doctor's office. It will probably cost a bundle, but by law they are your records and you have a right to have them.

Wherever you've moved to,your new doctor will be able to ask for permission for your files from your old doctor in Spain. Most all the medical records are kept with your doctor,as a copy is sent to them after any sort of treatment or operation

just get in touch with the offices of the drs and ask for them, apart from in exceptional circumstances they have to give you them they are entitled to charge a fee for copying etc though, usually no higher than £50
