You should be doing at least 20 minutes at a time for it to be effective. The rule of thumb is to do 3 20 minute workouts a week...so an hour is all you need in a week, not in a day...but if you are doing an hour instead of 20 minutes, good for you! But I would bump it up to about 22 minutes instead of 15. The real rule is that you need to be at your target heart rate for 20 minutes for it to be the MOST effective, though.

It's dependent on how intense the workout is. I've read in a couple of places that doing interval workouts can help to sustain a higher rate for a longer period of time without exhausting your muscles as fast. The problem with doing the 4 15 minute workouts is that you are going to need double the time that you would if you were to just do one hour of cardio instead. Your body needs time to cool down,you need to stretch before and after exercise, plus, you need time to warm up to your training heart rate.

You need to be in your target heart rate at least for 20 minutes to get an effect workout. So, if you can hit your THR for 5 minutes in 4 different increments, then it's a good day.

Kind of. You will typically burn more calories if you workout first thing in the morning before you eat anything. So, if you do one hour first thing, you might burn more calories than spacing it out through the day.

However, you body will start to adapt to the 15 minutes throughout the day and begin to increase your metabolism faster than just one hour blocks.

I used to do this all the time.
If it is high-intensity, the better.
You can even do 3 10-minutes sessions throught the day and it will be effective.

no your heart rate needs to be increased for atleast 20 minutes when doing cardio for it to be beneficial.

no, you should be raising your heartrate for about 30 minutes at a time to actually get benifit out of it.
