yes people who suffer extreme bouts of paranoia are considered mentally ill, unless it is due to street drugs, then it is called a drug induced psychosis schizophrenia usually always causes some paranoia anti psychotic medication usually relieves the symptoms

Well, if it produces any adverse changes in one's day to day life activities and they are not able to function because of it, then, yes. This needs treatment by a psychiatrist or psychologist.
