it's pretty clear with a yellow tint and normally has a mushroom slice in it.

theres nothing in it but little green things and a one thin slice of a mushroom

Miso soup, or miso shiru or sometimes restaurants will serve Miso if you order it, but if a soup comes with a meal, then usually they will just give you Dashi with a mushroom in it (no Tofu).

I am not sure if it is miso soup if it is clear. Miso soup is usually cloudy because of the miso paste. If it is absolutely clear, it might be Dashi which is a soup stock usually made with shiitake mushroom or fish.

yes it is called miso soup
it is made out of bean paste and the green stuff is chopped green onions and of course, mushrooms
although im shocked that you did not find any tofu in there
