I did this three times in high school. The first, the guy was making a to-do list, which I stole and added "Go to WPA with Sam" to the list. The second, he was more reserved and so I pulled him aside and said, "I don't know if you're interested, but I was wondering if you would like to go to Prom with me." That way, there's no real pressure. Third, I just asked him while we were hanging out.

The first two were semi-romantic, the third was just a friend. I only ever dated the second one.

Very few guys will say no without a valid excuse (i.e. already have a date, have to go out of town, etc.). If they don't have a good excuse, then they are jerks and you are too good for them.

Go up to him and blurt it out. Don't think of what you're going to say, just say it. If he says no, oh well. Ask someone else. Once you do it the first time, every other time is easy.
