Lab req: form the doctor fills out to tell the lab which tests to perform.
Lab rep: the results
Patient dismisal: sounds like a form your doctor would send you to dismiss you from their practice for some reason.
Medical History: form you fill out before being seen. Includes your history as well as family history
I've never heard od a charting form, but could be the worksheet the office uses to keep notes about their contact with you.

im not so sure about the rest but i know that a lab report is usually for a doc. they examine: blood pressure, blood sugar and stuff like that. so first they take a sample of your blood then they do a test filling out a lab report then give it to a doc so they can tell u if ur gonna die or live.
