it doesn't matter how long you practice and i don't think the person in the car with you actually gives a crap how long you practiced. if you make mistakes during the test, you are not going to pass. just do your best.

No, a state trooper or licensed examiner is not required to check for proof, when you apply for your roadtest, that you've actually driven a car (or any other vehicle) AT ALL--just a permit, plus the application and a willing sponsor. If you think you're God's Gift to the Interstates and can intuitively complete your roadtest without even an hour of real practice (other than video and arcade games), be my guest! I had a friend do just that (I was his sponsor). Let's just say it not only cost him the roadtest fee, but also a lot of embarassment!

I am not aware of any "Proof of practice time" but it has been awhile since I have taken any driving test. (with the exception of working tests...CDL)
The best thing that I can suggest is to get 4-6 months of practice with your temps before you go to the DMV. You want to be calm and confident when you have the tester with you.

I am not sure on exactly what you mean. You have to show your ability to drive but that is usually the only proof they look for.

It depends on what state you live in...every state has different requirements and also depends upon how old you are and you have to have had the driver education classes, usually. Go to aamva.org and find your state under Jurisdications to find out the requirements.
