Apply to the IRS for a tax ID as a religious organization. They'll give you all the requirements you need to meet, including incorporation.

suggest you contact a lawyer and find out the legal requirements in the State you reside in. this way your chances of doing everything properly will be higher than if you tried doing this yourself
