The W-4 form only gets put in your personnel file, it does not get sent to the IRS. SInce this form rarely changes, you shouldn't sweat havinbg a aform from 2001. If you want a 2006 form you can go to WWW.IRS.GOV and dowload your own form.
If you download the 2006 form, I doubt that HR will even notice.

There's no reason to refuse to use them; they really haven't changed. If you prefer to use a 2006 form, simply download them from the Internet and print them out. You can get them at irs.gov. It's such a little, tiny thing and it's so easy to get a new one. There's no need to make a big deal over it. If you refuse to use the old forms, your new boss might not have a very positive impression of you, and if you're already worrying about what you can and can't do (especially where paperwork is concerned) without getting fired, you might need to relax a bit before your first day. If you must use the 2006 form, simply explain that you'd prefer to download the current form from the IRS and hand it in.

New W-4 forms are not printed every year by the IRS. The 2001 form is legal. It is put in the employers file anyway.

I thought that as long as no changes took effect in law, with taxes. Then it is alright for the w-4 form to be from 2001. Keep a copy of it and take it to somebody like a lawyer, that is my second opinion.

The w-4 form hasn't changed. It only asks tax status and number of dependents. What matters is if your employers tax you correctly based on info you gave. No they shouldn't dismiss you they might laugh though. Realize they won't pay you until form is filled out.

You can just cross out the year, initial the change and use the 2001 form.
Since they require a W-4, yes they can dismiss you if you don't complete one.
