No, they will need an updated copy of the the w4 at the beginning of each year, but it is not neccesary to have current employees' fill them out again. The only time they may ask you to is if you want to change anything about your filing status or exemptions.

NO, new W4's are only necessary when you have a change in your dependent status or you want to change your filing status (single or married). I also changed mine to have additional taxes taken out with every check.

No, only if your situation changes. If you have children, get married, get divorced, and any other things that would change your status.
