Red Dog Mine in Alaska did get in trouble for lead and zinc dust coming off of ore trucks and getting into surfacewater and killing plants and fish along their haulage road.

Fort Knox, as far as my understanding, has been the most environmentally conscious mine in Alaska. If something was wrong with their reclamation plan or practices I am sure it was just a paperwork technicality. Regulators are famous for singling those out. I have personally visited this site.

Another instance was with the Pogo Mine in Alaska where a local Environmentalist group claimed that their SWPPP permit was worded wrong for discharging treated process water into a stream. Once again, it was a paperwork technicality and eventually they were allowed to contine business.
