Anyone youwill be sending an invite to.
Save-the-dates are sent out well before your invitations in order to inform guests of the upcoming wedding date and, in most instances, the locale.
Save-the-dates are nice if you have friends and family on your guest list who will be making a big trip to attend your wedding or ss taking place over a holiday weekend where it's necessary to book travel arrangements and accommodations well in advance.

Save the date cards can be sent to everyone you are planning to have on your guest list that is a non-negotiable. By that, I mean that people who will be there no matter how many changes your guest list will go through. It is particularly important that if any of the "non-negotiables" are from a considerable distance away, for example, they have to fly and could not theoretically drive to where you are getting married, that they receive Save the Date cards. Also, if you have anyone coming in for the wedding from another country that might require a visitors visa, they should also get the Save the Date card so they can make the required travel arrangements.

Save-the-Date cards are usually sent to everyone...your attendants should get a personal letter from you asking if they'd liek to be included in the wedding party. Bridesmaids, groomsmen, maid/matron(s) of honor and the best man(men) should definitely get a phone call or email asking permission...the parents of the flower girls and ring bearers should also get notice and permission.

Usually Save the date cards are sent to those who are in the bridal party and any out of town guests. I sent save the date cards to my husbands family that live in nebraska and his sister and her family that live over-seas. You need to send them far enough in advanced though.
