Go visit your local tailor. Most of them also make dresses. If not they may be able to reccomend someone who does.
When having a dress custom made make sure you get a contract. Make sure you have deadlines (when you go for a fitting, when it should be done, etc.) and specifics, what material its to be made of, a general description, especially of beadwork.
Another good idea, if you find a dress maker, see if they will make you a sample dress first. This is a dress made out of a cheap material such as muslin, that is made to generally mock your wedding dress. You will be able to try it on and see if it is what you want and what you want changed before the real deal is made!

David's, Vera Wang, and Aria definitely don't make custom dresses- they all have their own dresses that they mass market. They won't let you custom design a dress.
I had a dress custom made for prom and I found out about the seamstress from a small shop I was looking at for a dress- she said she designed and sewed dresses on the side so I asked her if she would be willing to meet with me.
So my suggestion would be to contact local seamstresses either buy looking them up in the yellow pages and giving them a call or just asking around at local bridal shops. They may be more willing to give you the contact info for an independant seamstress if you buy something else at the shop, like your wedding dress or promising to buy shoes or something. It's hard because they work on commission.
I feel your pain!!
