You address the letter as "Rabbi and Mrs. John Smith". The title "Rabbi" is in place of the "Mr."

First of all, it's spelled with a double B... Rabbi. Important to remember.
Treat the title that same as you would the title of doctor (Dr.)

Dear Rabbi Sandler,
Dear Rabbi Adam Sandler,
Dear Rabbi and Mrs. Sandler,
Dear Mr. and Rabbi Sandler, (if the wife is the rabbi)

Same as if you were addressing a doctor and wife, senator and wife, etc (not sure if rabbis can be women but if so it would be Mr. and Rabbi ____ )

It would just be Rabbi (and then their last name)

For example:

Rabbi Smith
