What kind of things does he like? Sports maybe? Music?
Make it a theme night- let's just say he likes football. Rent a football movie (The Longest Yard) or (Friday Night Lights), make some chicken wings, nachos or other "sports" related foods (or create a pizza together), buy a cheap, knock off jersey and wear it (it'll look really cute on you) and then give him a nice present- like a blanket with his favorites team's logo on it so you can cuddle up under it a watch the movie.
If you're gonna have sex, use a condom because next year you'll probably want to celebrate his birthday with just him and you- not him, you and your new born baby! Oh- forget the wine, it's over rated and you're under age.

Well, you could have a carpet picnic. Put down a blanket on the carpet and set it up as an outdoor picnic.
