I've had to shoot in hotel bathrooms before, to show the room to potential guests. If you go in the bathroom you can find a place in there where you can film without your reflection being in the mirror.
Or if your filming someone looking in the mirror, you just stand to the side and behind them, then zoom in a bit to crop yourself out of the shot
Oooor, they make a special set to get a special shot.

There are many ways of doing that.
One, if you take a close look at film cameras, you'll see they have a retractable chamber that could be holding the lens. If it is that way, you can use two functions of this to correct the image perspective variations, so If you are from one side, this thing corrects the image coming through the lens, by manipulating the angle with which light hits the film and exposes it. This way you can have the shoot and have it look like you are in front of the mirror.
The other could be just having the angle without correcting the image so it does have perspective.
The other is by deleting stuff in post, with help of diverse software available in post agencies.

First answer - also the background could be dark and they could be zoomed in from a ways back.
They could also camouflage the camera - in black usually.
The camera and operator could also be chroma-keyed - buy I doubt that.
