I've been very impressed with both Finnair and BA, but my favourite is Lufthansa.

From my personal experience, flying Air France, British Airways, Qantas, Cathay Pacific and Malaysia Airlines, I have to say that the asian airlines were the best in terms or comfort and service.
Singapore Airlines and Thai Airways have good reputation as well, but I have yet to fly with them.

Qantas is probably the best, the have an impressive safety record.
As for the worst air line, Ariana Airlines. You are not misreading when I say this, they have the most crashes on the ground. Their safety and maintenance record are so horrible that they have been dubbed scariana.

Best airlines are all of the major Asian ones (Cathay Pacific, ANA, JAL, Korean, Emirates etc.)They put Europeans and Americans to shame. Worst ones I've flown with are Easyjet and Ryanair, though lots of people use them quite happily. Not speaking from experience myself but I have heard horror stories about Aeroflot. and would never fly with them.

The Best: China Air
I recently flew to Bejing on China Air, they were curteous, efficent and could not do enough for you.
The Worst: KLM
I went to Venice Business Class with KLM, everything was too much trouble. For a Breakfast i got one cold Sandwich, nothing hot and this was business class. Pathetic.

The best has to be Emirates closely followed by Quantas for their safety record.
The worst must be Ryan Air with Easyjet a close runner up.

Singapore Airlines has to be the best. Good food, good service and decent fares.
Worst Long haul is KLM whilst worst short haul has to be ryanair.

The good scheduled ones are all fairly similar, BA, Virgin, Singapore, Qantas, Emirates.
However, the worst by some margin is American.
The best for domestic in the UK is BMI Baby, I use them at least once a month, always on time and you can pre book your seat and check in on line.
All charters are crap.

Virgin Atlantic is the best, so much space and so comfortable and you get a bag of freebies too! Air Jamaica was the worst I've been on, really unprofessional and the plane was so small for a Trans-Atlantic flight... Totally disappointed for the amount I paid...

Not only does it depend on where you are travelling, it depends on what you value.
What's important to you? Price? On-time? The comfort of the seats? The in-flight amenities? Frequent-flyer programs? Specific cities served? Access to airline-owned clubs in airports?

Quantas is the only major airline which has never had a fatal accident.
Aeroflot, on the other way, probably has more unused returned tickets than any other.
