A passport is a small book which carries personal information about you and it is used for travelling to a foreign country. You may not leave or enter another country without a valid passport. Depending on your nationality, you may freely enter and stay in a country as a tourist for a certain amount of time before you will need a visa. With other countries, depending on policy, relationship etc, a visa might be needed before you may enter into that country.
A visa is usually a stamp on your passport which allows you to enter, work or live in a foreign country. Some countries require this depending on your purpose of visit ( work, leisure etc)
You can check with the embassy of the country you intend to visit for visa requirements.

From http://travel.state.gov/passport/passport_1738.html :
"A passport is an internationally recognized travel document that verifies the identity and nationality of the bearer. A valid U.S. passport is required to enter and leave most foreign countries. Only the U.S. Department of State has the authority to grant, issue or verify United States passports."
From http://www.unitedstatesvisas.gov/whatis/index.html :
"If you’re a citizen of a foreign country, in most cases you’ll need a visa to enter the United States.
A visa doesn’t permit entry to the U.S., however. A visa simply indicates that your application has been reviewed by a U.S. consular officer at an American embassy or consulate, and that the officer has determined you’re eligible to enter the country for a specific purpose. Consular affairs are the responsibility of the U.S. Department of State.
A visa allows you to travel to the United States as far as the port of entry (airport or land border crossing) and ask the immigration officer to allow you to enter the country. Only the immigration officer has the authority to permit you to enter the United States. He or she decides how long you can stay for any particular visit. Immigration matters are the responsibility of the U.S. Department of Homeland Security.
There are two categories of U.S. visas: immigrant and nonimmigrant.
Immigrant visas are for people who intend to live permanently in the U.S. Nonimmigrant visas are for people with permanent residence outside the U.S. but who wish to be in the U.S. on a temporary basis – for tourism, medical treatment, business, temporary work or study."
With the visa, if you are traveling out of the country to somewhere else, the rules would be reversed and would follow that country's rules.

Basically Passport is something like a identity card, a proof of your citizenship, identity, age and address. This is obtained from your country authorities.
when you want to visit other countries you should obtain permission from that country's govt to visit their country, you will have state for what purpose u want to go there. Depending on their policies, your reason they will issue visa(could be tourist, business etc depending on what you have asked for). Visas are permission given by countries to you to visit their countries, they are normally time bound(say for a week or ten days or a month).
