If a patient had a kidney tx then a transplant nephrectomy 5 years later, does that mean that the transplant was removed? Also, what is the most common reason for removing kidney transplants?

It means that the implant was replaced with a new one. Transplant nephrectomy and kidney transplant is the same thing. And the most common reason for removing a kidney transplant is that the kidney that was donated was rejected by the kidney receivers body.

A Neprectomy is a removal of the kidney, I had one a year ago. The most common reason for a transplant removal would be the body rejecting the kidney.
